ENDODONTIC Treatment - FLORENCE

Endodontics is the branch of dentistry that deals with the internal tissues of the teeth, or the part that is not visible, the dental pulp, made up of connective cells, vessels and nerves.

Endodontic treatment is needed for teeth that have pulp diseases, resulting from penetrating cavities, trauma and fractures or as a preparatory treatment when making a prosthetic bridge or crowns on some teeth.

Pulp damage typically manifests itself with very intense, sudden and irradiated pain. The so-called “pulpitis” (the much feared “toothache”).

More rarely, however, the pulp becomes inflamed without any particular symptoms. Finally, even traumas, sometimes even minor ones, can lead to a dead tooth due to the lack of blood supply and nutrients inside the pulp.

Untreated pulpitis evolves over time into a dead tooth or pulp necrosis which can manifest itself with an abscess, found at the tip of the tooth with swelling and pain. It can also cause diseases commonly referred to as "granulomas".

Regardless of the cause that caused the original damage to the pulp, treatment is no longer limited a simple removal of the decayed tooth and carrying out a more or less extensive filling, but may result in endodontic treatment also called root canal treatment or devitalisation.

WHAT THE TREATMENT CONSISTS OF

Endodontic treatments are carried out after having correctly diagnosed the problem, after having taken one or more pre-operative peri-apical x-rays, and after having correctly isolated the operating field with the rubber dam, essential for obtaining an excellent result.

We then proceed by making a cavity on the chewing surface of molars and bicuspids and on the external or internal surface of incisors and canines. We then continue to removing all the decay-causing tissue, the contents of the root canal system and the micro-organisms that cause inflammation. From a technical point of view, we proceed in stages:

1) shaping - the canal content is mechanically removed using special tools and the canal takes on a conical shape

2) cleaning -  the organic and inorganic content of the endodontic system is chemically removed, using disinfectant solutions known as "irrigants"

3) root canal filling - the endodontic system is sealed with gutta-percha, a non toxic and inert material, together with a thin layer of cement, which will help prevent bacterial recontamination.

Most root canal treatments can be performed in one or two appointments. Endodontic treatment helps preserve the compromised tooth from cavities or trauma and helps avoid extracting it.

Following devitalisation, the tooth is more "fragile" than vital teeth due to the loss of substance. The more fragile it is, the more it is advisable to use prosthetic restoration (crown) to increase the mechanical resistance of the treated tooth.
